Independent Living FAQs

How will I know what level of senior care I will need?

The GreenFields staff is educated and trained to assess the needs of a future resident and determine the appropriate level of senior care a person may need. By contacting the Senior Living Department at GreenField Manor at 716.684.8400, you can discuss your needs and schedule a time to meet to better evaluate the appropriate level of care.

I am having surgery and will need rehab afterward. May I set this up ahead of time?

Yes. GreenField Health & Rehabilitation Center encourages pre-registration for patients having elective surgery, giving patients the chance to tour the facility, ask questions and expedite the admissions process. Call our Admissions Department at 716.684.3000, ext. 303, for more information.

What happens if I need more senior care in the future?

Residents receive priority access to all levels of care available on The GreenFields campus. Independent living in residential senior apartments is available at GreenField Manor. Assisted living in private senior apartments is available at GreenField Court. Memory care and enhanced assisted living are provided at GreenField Terrace. GreenField Health & Rehabilitation Center provides long-term skilled nursing, subacute, Alzheimer’s care, dementia care, and respite care, as well as state-of-the-art inpatient and outpatient rehabilitation services, including aquatic therapy.

Do you provide transportation?

Yes, we provide scheduled transportation to area doctors within a 15-mile radius, as well as planned activities and shopping.

Do you offer activities?

Yes! Our Creative Arts Department schedules many events and projects based on ideas and suggestions from residents. Current activities include concerts, scheduled speakers, exercise classes, craft projects, excursions to plays and concerts, picnics, parties, Bible studies, card groups, book clubs, opportunities for worship, our own art gallery and gardening.

What if I have a house to sell?

Many residents do have houses to sell. Since there is no upfront fee at GreenField Manor, it is not uncommon for people to reserve one of our senior apartments without having to sell their house first. The GreenFields offers a move-in coordination program to make the move as easy as possible.

Enhanced Living FAQs

Get answers to your questions about life at GreenField Terrace. 

What is enhanced assisted living?

Enhanced assisted living is a level of care which is between traditional assisted living and skilled nursing. At GreenField Terrace, residents can receive assistance of one person to transfer such as getting in and out of bed, in and out of a chair, or on or off the toilet, as well as help with medical equipment and urinary incontinence care.  A mechanical soft diet can also be provided. Residents in enhanced assisted living cannot have any significant memory loss.

What if my loved one needs the services of enhanced assisted living and also has cognitive impairment?

GreenField Terrace is licensed to provide memory care enhanced assisted living together. In memory care, we can also accommodate those who need assistance with transferring and assistance with medical equipment for an additional charge. The senior living staff at The GreenFields can help determine the most appropriate level of care that is needed.

Memory Care FAQs

Get answers to your questions about life at GreenField Terrace. 

Why would my loved one need memory care assisted living?

It may be time to consider memory care assisted living if you find your loved one has short-term memory loss, such as not being able to remember what he or she had for breakfast or not oriented to person, place or time. If you find they are not self-directing or need reminders for basic tasks to get through their day, such as time for meals or to get dressed, if the person is wandering or has exit-seeking behaviors, or if they are confusing their days and nights, it may no longer be safe for them to be living alone. 

At GreenField Terrace we can accommodate those with mild to moderate memory loss. The doors are secure to keep those who are exit-seeking safe and there is a beautiful courtyard for those who may have wandering behaviors.

What if my loved one needs the services of enhanced assisted living and also has cognitive impairment?

Subacute Rehabilitation FAQs

Get answers to your questions about the subacute and inpatient services offered at GreenField Health & Rehabilitation Center.

Who decides where I can go for rehabilitation services?

Your physician may recommend a facility, but the ultimate decision is yours. Be sure to tour as many facilities as you are able before making your choice.

At The GreenFields, we allow prospective patients to reserve a room prior to surgery which eliminates making an uninformed decision in the hospital. We will assist you with all of the necessary paperwork.

What should I look for when choosing a rehabilitation center?

1). The level of experience of the rehabilitation staff: Ideally, the average years of experience in the department should be more than five years, and therapists should have varied clinical experience. 

2). How often will you be seen in rehabilitation: Every person’s therapy program should be individualized. The facility should offer their services at least twice a day, seven days a week.

3). Types of services that should be available: Physical, occupational, and speech therapy should all be offered. Check to see if the facility offers specialized services, such as aquatic therapy. Also, ask if the facility has a medical director of rehabilitation services who oversees all programs and protocols. 

4). The facility itself: The facility you choose should be clean and appealing. Staff should be friendly and helpful. The quality of food should be very good, and you should have a variety of choices.

How long will my rehabilitation take?

Length of rehabilitation depends on your individual progress. Our patients and their families help plan their rehab program, and discharge goals are established at time of admission. At The GreenFields, the rehabilitation team meets regularly to review each patient’s progress and assure that everything is on track to meet the goals that have been established.

What should I bring? What should I wear?

The facility should provide basic toiletry items, like soap. You should plan to bring any special items you may prefer to use. Plan to wear comfortable clothing, like shorts, sweatpants, T-shirts, etc. Everyone should bring non-skid tie shoes or sneakers. If you are having knee surgery and don’t like to wear shorts, bring loose-fitting sweatpants that that will come up over a swollen knee.

What does discharge planning include?

Discharge planning includes:

  • Establishing the date and time of discharge
  • Determining the location of the discharge
  • Determining equipment needs: the rehabilitation team should make recommendations regarding equipment that may be beneficial, and order the equipment for you, if you choose.
  • Follow-up services: these services should be discussed and ordered for you, if you and your physician approve.
  • Rehabilitation staff should assure that all family members are trained in safely assisting you at home and will answer any questions you may have.

How is my surgeon or physician involved in my care?

Upon discharge from the hospital, your surgeon or physician completes a summary of your care that includes directions the facility will follow to meet your needs. In the event that there are any questions in providing your care, the facility should have a system in place to communicate directly with your surgeon/physician.  

Do you provide outpatient services?

If the facility you choose offers outpatient therapy services, you will greatly benefit from the continuity of care if you require these services. There is great benefit in being able to continue working with the same therapy team as an outpatient as you did when you were an inpatient.

What is aquatic therapy?

Aquatic therapy is rehabilitation that is done in a heated pool of water. Individualized exercise programs are developed by physical therapists in conjunction with your physician and according to your needs. The benefits of aquatic therapy are many. Buoyancy allows for easier joint movement. Water supports the body and reduces stress to all joints so muscles can be toned and strengthened during the healing process. The heated water allows you to complete your exercises in a pain-free environment.

GreenField Outpatient Clinic & Wellness Center in Lancaster offers physical, occupational, orthopedic rehabilitation, and speech/language services. Our physical therapy staff offers a wide range of services to assist patients in injury prevention and pre-surgical treatment, as well as traditional rehabilitation programs.

What should I bring to my first appointment?

Plan to bring an official form of identification, your insurance information, your prescription and/or referral, and all completed forms.
