Continuing Care Communities Offer the Best of All Worlds

Retirement is what everyone works toward throughout their productive years, and each person will have different needs as they grow older. The wear of age, progression of disease, accidents and injuries, and many other factors can dictate your lifestyle in retirement – and even your residence.

Independent living is not always sustainable as medical needs increase or impaired mobility means a new need for assistance with daily tasks. Traditionally, as a person’s lifestyle changes, they have needed to move to a specialized facility, like a nursing home, an assisted living facility, a long-term care facility or a dementia care home. However, at a continuing care community like The GreenFields, located in Lancaster, just outside of Buffalo, residents can stay in the same comfortable setting no matter how their lifestyle and needs change.

As your needs change in life, your life at The GreenFields Continuing Care Community changes with you. Once an individual resides at The GreenFields, they receive priority access to the full range of services and care options available on the campus. Let’s explore the different levels of care available and learn how life at The GreenFields offers support through each of the different needs of retirement.

Independent Living

Many residents move to The GreenFields to take advantage of independent living options. Upon moving to GreenField Manor, new residents can enjoy all the amenities of their new community apartments. One- and two-bedroom apartments are available, with some floor plans offering balconies or patios, as well as designs with fireplaces. Housekeeping and maintenance is also included, allowing for residents to worry less about chores and more about enjoying retirement. All buildings also have central elevators and an emergency call system, providing easy access and rapid response in case of any medical emergencies.

On The GreenFields campus, residents can make use of the outdoor spaces, including a gazebo, pond and walking paths, or grab a book and read in the community library. A beauty salon and barber shop are available, as well, along with an arts studio and fitness center. Restaurant-style dining is popular with residents who like to dine with friends or loved ones, and Grace’s Place Café offers snack options and lighter fare outside of meal hours.

Assisted Living

In GreenField Court, residents can continue to enjoy many of the same benefits of community life, but have access to assisted living services to provide additional support and aid with day-to-day needs. Assisted living is not a nursing home – it’s a setting where residents can still live independently, but receive help with medical needs or mobility assistance. Residents living at GreenField Court can take confidence in knowing they can receive the support they need on a long-term basis without having to give up any of their independence.

Needing help isn’t always easy, but The GreenFields works to provide assistance in a way that respects each resident’s lifestyle while aiding with daily activities. Nursing staff is on site 24/7 to help with a wide range of needs for residents, and provide peace of mind that if they require assistance, it’s always available. Residents in GreenField Court can also coordinate transportation to local venues or doctor’s appointments throughout the Buffalo area.

Enhanced Assisted Living

Individuals with greater needs can take advantage of GreenField Terrace, which offers enhanced assisted living services for those with a greater need for aid. This building is designed with a central garden courtyard, and serves as an in-between point for residents who need more help than assisted living, but do not require skilled nursing care.

Skilled Nursing

Residents at The GreenFields’ campus in Lancaster also have access to skilled nursing services at GreenField Health & Rehabilitation Center. Skilled nursing services allow residents to receive the specialized care and attention they need while still enjoying a homelike lifestyle.

Skilled nursing care is tailored to each individual’s needs, allowing residents to receive the necessary care while still enjoying the benefits of community life. Friends and loved ones can still enjoy time together on campus while getting the medical care and assistance needed.

Rehabilitation Care

GreenField Health & Rehabilitation Center also offers a convenient location for individuals who need rehabilitation therapy following an illness, surgery or accident. Following a joint replacement or broken bone, therapeutic rehab is often a big part of restoring not only a person’s health, but their mobility. At the center, residents can participate in occupational therapy, physical therapy, speech and language therapy and even aquatic therapy in The GreenFields’ full-sized pool.

Residents on The GreenFields campus don’t have far to travel to get to their rehabilitation appointments, either, and, depending on their needs, can schedule visits to the Rehab Center while living in their campus apartment. Having this easy access to rehabilitation services can help improve recovery times and outcomes for patients, too.

The center also provides outpatient rehabilitation services not only to The GreenFields’ residents, but community members from across Buffalo and Western New York. In addition, staff at The GreenFields have received QAPI accreditation and Five-Star Quality Rating by the Centers for Medicare and Medicaid Services, speaking to not just the high quality of care, but also the positive results of the rehabilitation team.

Memory Care

Finally, residents at The GreenFields also have priority access to the three memory care buildings on the Lancaster campus, making the transition from apartment life to dedicated memory care a simpler and easier process. In many traditional environments, dementia care is seen as a specialized service and cannot be accommodated in a more standardized nursing home environment. At The GreenFields, however, residents can easily transition from residential or assisted living apartments into the community housing at GreenField Terrace.

Each home setting features 12 rooms for residents, which helps to allow for individualized attention and care. There is also central shared space, including a living room, sitting area, library space and access to the shared outdoor patio. GreenField Terrace also provides more freedom for residents, and uses less of a structured schedule environment and more freeform options for residents to participate in activities or dine when it is convenient for them. Staff is also on site 24/7 to provide support, administer medication and respond to emergencies, while the building design also works to remove the risk of wandering, helping to ensure safety and security for residents.

Life Changes – So Does The GreenFields

By moving to The GreenFields from Buffalo or anywhere else in Western New York to enjoy retirement, residents receive so much more than simply a place to live. While downsizing is common in retirement, finding the care needed as life evolves isn’t always as simple. With priority access to all levels of care at The GreenFields, there’s no need to move from a retirement community to a nursing home to a dementia care facility, going through the stress of finding a place and moving time and time again.

Continuing care means just that – care that continues with you as you continue down the road of life. Wherever that road takes you, The GreenFields offers the support, care and assistance needed and the community to enjoy all the years of retirement.
